R8 PRIME is a telehealth platform and pharmacy facilitator. We facilitate access to telehealth services provided by independent licensed healthcare providers. R8 PRIME:

• Does not practice medicine

• Does not guarantee that any patient will receive a prescription

• Does not make clinical decisions

• Does not guarantee specific health outcomes

• Is not a substitute for emergency medical care

3. Consent to Telehealth Services

By using our platform, you consent to receiving telehealth services from independent licensed healthcare providers. You acknowledge and understand that:

• Telehealth services involve the delivery of healthcare services using electronic communications

• A synchronous (real-time) telehealth consultation may be required in certain states before treatment can be prescribed

• Telehealth services may not be appropriate for all medical conditions

• You have the right to refuse telehealth services and seek in-person care

• Service availability varies by state and jurisdiction

• Not all patients qualify for treatment through our platform

4. Prescription Medications

All prescription medications require a valid prescription from a licensed healthcare provider. Eligibility for prescription medications is determined solely by independent licensed providers following clinical review. Completing an intake form does not guarantee a prescription. Compounded medications, when prescribed, are not FDA-approved, and the FDA does not verify the safety, effectiveness, or quality of compounded drugs.

5. User Responsibilities

You agree to:

• Provide accurate, complete, and truthful health information

• Be at least 18 years of age

• Use our services only for lawful purposes

• Not misuse or attempt to circumvent our platform

• Seek emergency medical care (call 911) in the event of a medical emergency

6. Disclaimer of Warranties

Our services are provided "as is" without warranties of any kind, express or implied. We do not warrant that our services will be uninterrupted, error-free, or that any specific health outcome will result from use of our platform. Results vary. Individual outcomes depend on many factors.

7. Limitation of Liability

To the maximum extent permitted by applicable law, R8 PRIME shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability shall not exceed the amount paid by you for services in the preceding twelve months.

8. Governing Law

These Terms & Conditions shall be governed by and construed in accordance with applicable federal and state laws. Any disputes shall be resolved through binding arbitration in accordance with applicable arbitration rules.
